omen of amelioration bug

sometimes the omen dose not work while maping and I lose the 10 % of exp

like today I had 9 active
the first die it worked
the second die it worked
but the third death cost me 10% of exp and the omen did not work

dose it happen to you as well
is there a hidden role or it is a bug in the game



P.S: I don't have any other omen active or even in my inventory

As i understood it work only once per map. If the third death was on same map than second death than it's normal.
You can see it in the description of the omen.

If it wasnt on the same map than i dont know and it might be a bug i guess.

I found this description on omen of POE 1 but I am playing the item on POE 2 where that was not listed in the description


POE 1 item Omen of amelioration
Will be Consumed when you Die
to prevent 75% of Experience loss
Each player can only consume one Omen in each combat area.

Only one Omen can be triggered from combat in each instance.

yes, it says it is on combat so, you can't use two omen on combat and both will be activated.

but one area are not mentioned here.
