Treasures of Utzaal Bugged

I have the quest to clear the molten vault, kill the boss 3 times, even states to pick of "The Hammer of Kamasa" but it doesnt drop.

I have spoken to Oswald at the Ziggurat Encampment, but I can't continue without this quest being completed.

I am going to try to drop the quest and retry again, but this is wack.

Any ideas?
