BUG REPORT, I can't get Hammer of Kamasa from Boss (ACT3)

I don't know why this happpened, but when i finished boss. They didn't show anything about QUEST Item, they just show a low-quality of items and a bit of gold. i hop u can fix it, because i really piss off when i see sub-quest still on my right screen.
