Ritual altars not spawning on maps with rituals

Been running a few maps with ritual altars on and i have noticed 2 times now that some times ritual alters dont spawn inn even tho it says on the node on the map that in contains ritual alters. The first map i noticed this on was on a map with delerium, breach and ritual altars, but i have run a few maps now with breach, delerium and ritual altars on the same map and have not had any more problems. The second time it happened was on a map with abyss and ritual alters and no ritual alters spawned. Have fully cleared and explored the maps to make shure im not just blind.

Happened to me twice, also. Both maps had breaches and the ritual tag, but didn't show any altars.
