Extra content in maps missing

I had multiple times now that a map in end content shows me that it contains for example rituals but no where on the map they can be found. It also shows the icon in the to be completed list.

I am currently in a Forge map and have cleared the whole map which should contain a breach, abyss and ritual altars. The first two were there and i cleared them. But no ritual alters, so i do not get the green check on the icon.

That is not the first time i observed this.

Edit: and happened again witth ritual alters.

And again, but this time the map should have ritual and expedition but had none of them. Map type is Sulphuric caverns.
