Sanguine tides sometimes doesn't regain flask charges

Sometimes, sanguine tides will not recovery flask charges. Cause is still unknown
I have had deathless maps before where this occurred so I doubt it had something to do with dying. Also going to hideout and back will fix it.

Potentially useful info: my gear

Flask 1: 26% instant recovery, 56% increased charges gained
Flask 2: 84% increased life recovered, removes 15% from mana, 62%; increased charges

Belt:
20 of flask life recovery applied instantly
5% increased flask life recovery rate

Gameplay:
I just spam flask 1 while doing spells and stuff
Frequently am weapon swapping, orb of storms on 1 weapon set, ember fusillade on the other
Dying can often break Sanguine Tides, but once it stopped working without dying in the abyssal depth. Might have something to do with "prevent recovery" mobs?
Same problem most of the time for me is dying in the map and re-spawning to checkpoint with my friend and other than that it seems the Turn off flask recovery can bug it too
I'm also having this problem in maps after I die. I could be wrong but it seems more often in delirium maps.
I'm having this problem as well. Seems to only happen in maps after dying.

I need to go back to town and drink from the well, upon returning it works again.
