Ok, It seems as long as any big ticket streamer faces this problem and makes it obvious and public, GGG wont acknowledge this bug and add it to 'Known Issues / FAQ' section.
So i literally added every person from reddit and poe forums to my friend list who faces this problem and talk with them. It seems the 4 keystones you put points into on the left bottom of the atlas tree makes you get ' ONE '' of the keystone nearly permanent.
Solution: take 2 of the keystones randomly and try every combination. One of the combination (unique to every player dunno why i made my friend try my combination of 2 to see if it gives same result which is slay all and it was giving stone circles to him but different 2 combination give him survive' will give you permanent ultimatum mode of your liking. I am now getting slay all nearly %95 of the time and now this bug is actually a good thing.
So GGG fixes it or not at least now there is a solution.

Other Bug reports seems to be noted directly and getting an answer by GGG but they seem to ignore this bug. There are so many threads about it now and not a single one got acknowledged.

I tried every combination but never got only survive :(
Now im stuck with protect the altar which at least is better than slay all monsters but this still sucks ass.
I hope ggg will doe something soon.

I tried doing this. It worked for a few maps and then it goes back to stone circles for me. Depending on which combination I choose it switch to another option, but only for like 3-4 maps and then it goes back to the bugged one.
It seemed to unbug without using scarabs. But with scarabs it bugs again.

Sadly did not work out for me.
Tried every combination of 2 of them. after that i picked a single one of them still doesnt work.
Maybe its map relatated ..
Or did someone of you guys tried it with another char?
I dont have a second one.

mostly getting kill monsters. maybe in 20 maps im getting one stone circle which im fine with when you thing on original settings i should get 1/7 chance to have one. but as i said before i made my friend test this combination and he was getting capture stone circles a lot with it. I really dont know whats wrong with this ultimatum bug. but im sure its about this part of the tree.
