Omen of Amelioration sometimes doesnt work

I died once on a map omen worked but on the same map died 2 times and doesnt work . I lost %20 exp . Has anyone experienced this situation ?

can confirm that. i died, lost 10% experience, the omen of amelioration is still in my inventory, not gone

you can only have 1 omen activation from battle per map. it says it in the tooltip
if any omen is used, there will be no other omen activation in the same map. |

Also happen to me today, lost 10% with omen still active in inventory.
