Omens not being consumed correctly

The only 2 I've tested this with are Omen of Amelioration & Omen of Secret Compartments. I'll have either or both activated in my inventory, but about half of the time when I die the Amelioration one doesn't get consumed, no message in chat, and I lose full xp. Also the strongbox Omen sometimes won't get consumed to allow me to open a strongbox twice.

I'm not sure if there's some kind of conflict with having multiple Omens in inventory or having multiple "active", but something weird is going on and they're not always getting consumed even when "active"

Only one Omen can be consumed per combat area. This restriction is listed on the Omen.

Something wrong is still happening here. I had 2 rare strongboxes... I had a strongbox omen and an experience omen in my inventory... I deactivated both... moved the strongbox omen into the "first slot" of my inventory and activated it. The EXP omen was not activated. I clicked the strongbox... killed everything... loot drops... strongbox disappears... omen doesn't get consumed.

Then I go to the 2nd strongbox... same exact thing.

Then I activate my EXP omen and go to fight the boss... I die with the EXP omen activated... doesn't trigger and I lose xp.

THEN I go back to reattempt the boss... die again... THIS TIME the omen gets consumed and I save 75% exp... I literally didn't even touch the omen or reactivate it or anything... it was activated on the first boss attempt and didn't get consumed... then randomly on the second attempt it did work.

I still maintain something's not working right here
