Omen not triggering

Omen of amelioration was active in my inventory (right clicked, red borders visible). No other omen even in my inventory, got 1 shot by a mob twice in a row (respawned at the checkpoint next to it mid swing animation, lovely) but the omen was still active in my inventory after both deaths, 20 percent exp down.

Nah , probably you Had already another Omen proc in that map before...only 1 Omen per Map ... Not 1 Omen of a kind... Probably u used a duplicate Strongbox or something?!

Nope, was the only omen i had in my inventory for a very long time, i really never use them other than have the omen of amelioration in my inventory, have only used omen of corruption once this whole league. Trust me i thought this through, no other omen was used. Also ive found similar posts of people having the same issue. It seems rare though

This happens to me 3 out of 5 times I die. Omen is activated but doesn't always get triggered. I die... check inventory and It's still there & still activated.

Buggy AF!

Just happened to me today...Died twice because I suck, but the omen was still in my inv lit up and XP went bye bye
