How to unlock 3rd slot on lost tower

Why i cannot unlock 3rd slot with 6+ modifiers on map ? Its bug or im doing something wrong ? Screenshot with my map: https://ibb.co/XRpLRSB

It is a 5 mod map. 200% mods count as one, 40% as lightning is 2nd, 8 pacs is 3rd, 57% gold is 4th, and 42 % rares is 5th. In 6 mod maps you will have Revives Available 0, not 1.
