Visual Soul Core Bug Preventing Progression

Hey,

First, I record a video that show the bug so you can easly see for yourself.

So, I encountered a strange bug related to Soul Cores and map progression:

It probably just looked like I had a Soul Core on my back visually, but the game refused to let me place it in the stone altar.
There was no Soul Core available to pick up.
I don’t recall picking up a Soul Core earlier.
In the video I recorded, you can see that there is only one switch that unlocks another part of the map. However, in the newly unlocked area, there is nothing—no Soul Core or way to progress.

The video does not show any monsters because I had already defeated them.
