Abnormal Disconnection - Error: Failed to join any instances

My game just crashed during a map, and I now cannot log back in to my character, whenever I try I get sent back to the log in screen with the message: Abnormal Disconnection, Error: Failed to join any instances. This persisted for about 5 minutes, didn't appear to be an issue with my internet as all other internet functions were working fine. Eventually managed to log back in and my map portals were gone.

I'm not up to maps yet. Just got the game a few days ago and have played without issue. But, for the past few hours, in various areas in Act 3 I have had freezes and disconnections. I was booted from the game and failed to log back in several times with the same message about the 'Abnormal Disconnection', as well as errors about not being logged in. I'm on the Steam client.

I had the same error message trying to portal back to my hideout, but didnt get disconnected.. After a few attempts I was able to get to my hideout, but my portals had closed and my map bricked..
