Unable to receive +40 spirit from Cruel Act 3 Azak Bog Witch quest

Killed the witch initially and managed to submit the spear to complete the quest to get the charm. However, did not get the +spirit quest item. Tried killing the witch 3x but no drop.

Same here, level 97 Witch and have never been able to complete this quest.
