stampede and leap slam keeps breaking.

running Maps and stampede and leap slam keeps breaking. it works great and then all of the sudden it won't work. i have to spam sunder a few times to clear the packs but need to run away. the only 2 things that i found that fixes it is re socketing the two abilities (MID FIGHT LOL) in skills tab or teleporting out and coming back to the area but that only works until your run out of portals and thats not good thing for farming. sat for a minute or two to make sure and nothing. re plugged mouse and KB nothing. its only these two moves everything else works. its ruining all my breeches and delirium runs :(

so far its consistent with roll spamming away from grenades for example and as of recent is also breaking after ancestral warrior totem. about 4 times mid run with in a 5 minute time span. I checked to see if there were any ailments, debuffs / curses or stuns but saw nothing.
