Ritual tp bug & Omen´s not working

1st bug:
Rituals randomly teleporting into the middle even if not stuck or out-of-area

2nd bug:
Omen on death effects are like coinflip, sometimes omen of resurgance works and sometimes it doesn´t. Doesn´t matter if you get insta killed or you die slowly, it just decides not to use.
I´ve also encountered 2 omens getting used in one map, but sometimes not even one procs, if needed to.
