You can find information on the Twilight Reliquary Item Design here. If you wish to submit multiple times, you'd need to purchase the pack multiple times. Effectively, you'll be able to choose a unique item you've found and add it to the reliquary pool of items along with a message, and then when someone finds the Twilight Reliquary they have a chance of receiving the item you selected and see the message you submitted. It's a way to leave a permanent mark on the game as a thank you for your generous support.
Guild-transferred cosmetics will still be available on your personal account. Your purchases won't be reset - they're tied to your account, which encompasses both Path of Exile 1 and Path of Exile 2.
