Xyclucian, the Chimera, Chaos Temple Loot POOF

I killed Xyclucian, the Chimera and it dropped the Chaos Temple green quest item.

My inventory was full and it just vanished. The quest is in a state that I have the quest item.

I then proceeded to kill the boss again and it did not drop quest item.

Class Sorceress, Chronomancer level 37 Hardcore Solo Self Found
Frost Bolt + Cold Snap

Please advise on how to obtain quest item:(
