Unable to perform second ascension using quest Trial of Chaos

I completed the 1st ascension using Trial of Sekhemas. After I killed the boss for Trial of Chaos and took the rewards, the game crashed when trying to walk past the altar with the rewards.

I was able to log back into the game into the same Trial of Chaos instance. The game showed Trial Complete when entering the Trial of Chaos. However, after running to the end of the Trial, the game crashed again when trying to run past the reward altar. This happened consistently each time I tried it.
