Give us a death log

"Dying without knowing why" is a meme in PoE.
Give us a death log, please.
I'd like to know what knocked me out, especially when I drop dead from 10k Energy Shield + 2k HP without anything hitting me.

Yep, a deathlog would help a lot to identify whats wrong with a build.

I skilled into "Chaos Inoculation" (immunity to chaos damage but max life is 1), "Eldritch Battery" (ES is converted to mana) and "Mind over Matter" (damage taken is fully absorbed from mana). With Everlasting Gaze (unique amulet) I still have a pool of like 1.5k ES.

Currently I am dieing with full ES and I don't know why. There are mana flasks wich sacrifices life to recover a higher amount of mana, but I don't use these. I have no clue why I take damage past my ES.
