iron citadel completed but says 'map failed'

completed the iron citadel. i did recieve the quest item but it still says map failed on the atlas.

same issue. got the reward but can't go through

had this happen twice, completed citadel, turned in fragment, citadel map was stuck "in progress" until eventually it failed, bricking that part of the atlas.
