Quest item dissapeared (Trials of Chaos)

Killed Chimera boss in Act3 (Xyclucian, for first time), inscribed ultimatum (green quest item) dropped, picked it up, went to town, talked to merchants, and it is no longer in my inventory. Cannot progress in quest (wants me to place the thing in). Killed Chimera for the second time, quest item didn`t drop. Stucked.

Update - it might be (and i`m not sure, because it should NOT be possible) that i accidentally disenchanted the green quest item. I`m not aware of it, but where else can it be. Fellow player gave me another regular non-quest ultimatum, and the quest works with that.

however, after dying i cannot repeat. I need the quest item back
