error:swap_chain->pressent() fail . ruins the game for me. (crashes every 5 min.)

hi i have this crash like every 5-10 min at its ruining my game experience quite a bit. i cant accend becouse of crashes vs the boss and it reseting the boss hp but not my honor so i die to lack of honor. this was just a thing i noticed the crashes had a huge impact on. also act 2 caravan trigger this crash more intensified. im sure there is many others who have this problem error:swap_chain->pressent() fail. and its quite annoying.
