Missing passive skill points

I completed quest "The lost lute" in Act 1 cruel. I got item which gave me 2 passive skill points and I used it. Then I went to hooded one, and refunded 3 points. I got those 3 points, but 2 from quest dissapeared. It kind of broke my character now.

Edit: Tried to find lute again, but obviously there was none, because quest was accomplished. Restart of game didnt worked either.
