I honestly don't know what happened to a quest item. Please help possible bug.

I am on the second round of act two (beat act 1,2,3,1B) and I had both of the relics for the quest "Ancient Vows". I also had a quest that told me to possibly do my 3rd ascendancy run. Well, I went to the trial (apparently in the Normal act 2 even though I went underground in the harder act 2) and prepared to throw some of the items I picked from killing mobs in the stash. Well inside the room with the ascendancy alter I accidently threw the Kabala Clan Relic on the ground (many weird issues with stash and throwing stuff on the ground in console btw) and went to back out of the menu and pick it back up. When I did such, it was no longer on the ground and appeared to just vanish. I then check my stash and the relic stash box. and no luck. I go back to keth and slay monsters to get it to drop and no luck. I appear to have made a quest item disappear.

I am now 59 due to me attempting to refarm the relic.
I am on the PS5 and have put in a pug report there as well

+1 exact same issue. Dropped it in the trial because of the issue with the stash in trials (press Y on xbox to stash, only drops items on the ground).

Tried to run bone pit again for the one I am missing and it will not drop again for me.

+1 exact same. Was somehow able to hit “stash” button in Temple of Chaos on my quest item, now the clan relics for Cruel T2 are gone forever and won’t drop.
