Offerings not spawning

When using an offering they sometimes don't spawn in. The skill consumes the mana but the offering just doesn't show up.
It's not a case of it spawning and immediately dying, it simply just doesn't spawn at all.

Same for me. I have been using pain offering for half of the campaign, and it happens every 2-3 packs of monsters. I noticed that it always spawns when there are no mobs around, but in packs of monsters they sometimes just don't appear. I only've seen that on pain offerings though. I have been using bone offering too for a few hours, and it always spawns, but pain offering still doesn't sometimes
