Quest Trial of Chaos

Hi, i got killed the Chimera and it's dropped me Inscribed Ultimatum for trial but once i went to Jiquani's Machinarium it's disappeared, second kill doesn't give this item anymore, is it fine or i am probably missing 2 points forever?

have the same problem and im locked out from progress, only that the thing didnt drop for me at all and yet obelisk implies i have it and want me to put it in.

Expected an npc there but there was noone just the obolisk requesting to insert that thing.

Did you find a fix?

Ye, looks like you can complete just regular Ultimatum it still gives u ascendancy points
