PoE2 - Reporting two bugs with video - Falling through map & two Act 2's showing on UI

For the first bug during boss fight my character did eventually TP back up top but I did not clip it I guess.

The second bug, I was in Act 2 and I did /hideout. Saw the NPC with quest marker so I talked to him. After that as you can see in this video I saw what I believe is the 2nd round of the Act 2.
