My quest inscribed ultimatum has disappeared and im afraid im soft locked

I beat the boss right before Jiquani's Machinarium and my quest updated to put the ultimatum in the shrine but When I emptied my inventory into my stash I think I accidentally clicked it and now its gone. I have restarted my game and killed the same chimera boss three times but it hasn't dropped and I dont have it in my stash or inventory.

I am having this exact same issue unfortunately :(
