Ancient vows quest, Cannot insert 2nd relic

There is no way for me to insert the 2nd relic to complete the quest. The first relic i was able to insert with no issues but now I go there to insert 2nd and the ui is completely gone. I have tried restarting game, tping to town, swapping to controller because someone said that worked for them(it did not for me) nothing has worked
