Ascendancy Quest Item Loss Bug

What happened: I was doing the ascendancy quest, after failing the first time I got the quest item back to retry, my second attempt was "successful," I managed to beat the boss but I "lost" afterwards due to the field damage, and this time I did not receive my quest item back.

The quest itself currently says : Use the Altar of Ascendancy to choose your Ascendancy, but I lack the quest item to do so.

I still have another item to do another run, a grey one, and I'll check whether I can continue my ascendancy anyway, but I do think it is important to make sure other players don't lose theirs as I lost mine.

I'll edit this after I've finished the new run, to make sure whether I can still get my ascendancy points or if it is bugged.

EDIT : never mind me noob I failed again, so can't check.

TL;DR: Dying after beating the boss in the Ascendancy quest, the game does not give you back the item to do the Ascendancy run.

I am also stuck on this issue - completed first ascendancy trial, but cannot claim any reward/path even though the quest line is at that status. Room is not accessible. Went back in a second time and no joy still blocked.

I feel the same way, when will it be fixed?
