Ascendancy Boss Death Bug

I killed the ascendancy boss during the quest Ascent to Power, but right as I killed the boss I preceded to die. This gave me completion of the quest making the next step to use the altar of ascendancy, but the only option that I had was to start back at room 1. I do not have the quest item to start the trials again locking me out of using my ascendancy points and have no way of using them.

You have to speak to her again I am a dumbass.

I have the same issue where I can't get back to the alter after dying and killing the boss. When I speak to her there is no option to get to the alter?
