Omen of Return regularly just does not work

More than half the times I dies, these just do not open a portal. Not sure why or why sometimes it does work.

i'm going to assume that they require you have a portal scroll (though never used one)

Omens really need the subtext to inform players that you can only use one omen per instance.
Per map, 1 omen of any type can be used once. |
