Delve Haunted Remains Spawn Instance Failore

I am posting a notice for an instance error in the Haunted Remains Delve nodes. The reason being is that the event boss for the node fails to spawn even after all the mobs have been cleared. I have ran the node multiple times to the same effect. There is no error code to send. The only way to bypass this malfunction is to exit to the login screen.

The haunted remains delve node that spawns ancient phantasms is bugged after a recent patch. Its supposed to spawn a rare at the end thats the win condition but does not at the moment. I checked at lower depths and it does not complete there either.

Also seeing this issue, bug 2,655,756,975.

Haunted remains nodes in delve "activate", that is, you can click on the thing to begin them. But then they very very slowly spawn adds like one at a time. Then it just stops spawning, and the node never completes. This softlocks the game in a sense... you can't leave,portal out, etc. You either have to die or log out / swtich characters to get out.

This is the second one I've had this happen on today.
