An unexpected disconnection occurred

I get disconnected every 5-10 minutes, the game is unplayable at this point. The game kind of freezes but I can move my character but enemies/skills won't move/work. When it crashes me out to the main menu there's this message "An unexpected disconnection occurred". How can I fix this? I probably didn't deliver some valuable informations but I just don't know what should I send in here for you to help me. Please guys help me out, I really want to play this league but it just makes me wanna punch my monitor. Thanks
