Annihilation Approach boots randomly killing me instead of stopping at 1 life

Annihilation Approach Dragonscale Boots usually wortk life RF, that it stops and turns off at 1 life instead of killing me. But randomly it seems to kil the player instead. It killed me in hideout, start of a map, act9 aquaducts portal, no degens on ground or anything.
It might have something to do with bugged flasks early in map, where they dont work for like 1-2 seconds after leaving grace. As pathfinder player I know this bug is here for ages.
Also for example maps with reduced life/es recovery where with its scaling it is 98% reduction, I always die to the degen from the boots. Repeatedly, every time I turn it on in such map.
