Alva Incursion bug

When doing fourth incursion in a map I encountered a bug which caused:
1) I couldn't open the League Interface (default "V" button) when I entered the incursion
2) Doors to open in the Incursion (Altars of Passage) didn't display name of the room they led into, just "Altar of Passage".

I encountered this bug a few times already but now it cost me a Locus of Corruption, so I decided it's time to do something about it ^^ I see a couple of potential causes of the bug, however I don't think any of them occured every time I had this bug:

1) in the map I encountered Ritual and its league interface took precedence over Incursion interface - it flashed/opened for a second when I entered the incursion and pressed V, but after it disappeared pressing V didn't do anything.
2) Before entering the fourth incursion, I talked to Alva which opened the interface, then I left her, died, reentered the map, opened the incursion without viewing the panel and entered the Incursion, causing the glitch somehow
3) I didn't do Alva's quest during the campaign and game treats me like unable to do the content sometimes.

I have a video of the situation (5 minute snapshot, not the whole map sadly), I can upload it to Youtube or transfer in any other way.

Bug report reference number is 1 954 312 898, although I generated it after writing the whole post, so like 20 minutes after the bug occured, I don't know if that matters.

I encountered the bug again, it worked a bit different now. I again died after opening the league interface when talking to Alva and then opened the incursion directly without showing the interface. Now pressing V showed me the temple layout, but there was no diagram that is displayed on the right side, showing the outcome of killing an architect. Displaying room names on altars of passage worked this time. Have a video snapshot of this situation too if necessary.

I think you can remap ritual and Incursion buttons.....
