Version 3.16.0 changes:

"

Sources of Fortify now grant a number of stacks of Fortification based on the percentage of the enemy's ailment threshold dealt by the hit that applied Fortify to you, with a large bonus to fortification stack generation against rare and unique monsters.

Fortifying hits no longer grant a fixed strength buff for hit regardless of damage dealt. They now grant stacks of a buff depending on the damage on the hit, with 20 stacks have the same effect as the pre-3.16 buff.

It appears that your build is based around poison/damage over time, and the low damage of your hits means you don't gain any stacks of Fortify.
