Maven fight bug?

I just did the Maven fight for the first time and after a couple phases she killed me with her beam that stops regen. I am running an RF build and am still new to this game compared to all the no lifers. Saying that I am 100% sure that the fight bugged out for me because she killed me twice before the beam hit me. Once the beam hit me every portal I went back in with and used RF was an instant death...why? I have no idea I would walk in and put on RF the beams would not be anywhere near me and I already killed her minions so it would not be anything from them and no weird colors ground. I would walk in pop RF watch my hp and shield drops from nothing and die. Very very irritating and makes me not want to play anymore because its not easy for me to get that fucking writ.
