Am I correct in my understanding of how this is going to function?

From what I heard on the live stream, they said that it cannot be used on any item which has been modified - so the only items you can use this on are something that dropped ID'd?

Does using a Wisdom Scroll count as "Modifying" the item?

I think you misunderstood what they said. Here's a screenshot of the item: https://assets-ng.maxroll.gg/wordpress/Hinekoras-Lock.jpg. You can see that there's no restriction on applying the Lock itself, only that you lose foresight if it's modified in any way.
