Ping general failure only when playing POE

So I've been having these issues since 3.20 approximately: I can have a ping for 24hours to my router&modem and google dns and have 0 issues, including while playing other games. When I play POE however I get lag spike/dc for around 4seconds usually which on pings is really "general failure" as illustrated here https://i.imgur.com/ccMLkDz.png (around an hour before posting time). Issue that is captured in the screenshot happened when I shield charged into a wall and through monsters in a map. I'm on a wired connection, using netgear xr500 connected to fiber modem. I did all the usual network protocol resets, changed cables to new, everything I could find and also did the tweaks that intel advises to do on their cards(I'm on I-225V). Am I left only with windows fresh install + poe fresh install?

well either the router drops the port or the netcard does. That's the only reason for the "general failure" you get....

Can you connect directly to the fiber modem ?
I've found in the router log that it refreshed IP of my PC at that time. Is there a way to tell if it initated that refresh or did my card fail and that caused the refresh?
I cant connect directly to the fiber modem but my ISP will check if they can move it without my wife divorcing me for the cable on the wall.. For now I plugged in to brand new switch I got that connects me to ISPs modem and all other devies will use the router which is on another port of that switch.
I could plug in for a day for example but the issue is so inconsistent that I might not catch anything. I also got a USB 3.0 network card so that's probably next thing I need to test? Does it make sense just to reinstall windows to be sure?
