Xbox Instance Connection Issues

Ive been getting the message "Disconnected Failed to connect to instance" whenever to try to get in the game. It happens after I pick which of my characters I want to use, then it loads but send me right back to title screen with that message. It's been giving me this problem for quite some time now. I've tried changing gateways, networking modes, and every other "fix" that's been thrown my way. Can someone, anyone, please just give me a real solution for this.

I'm also having this problem. Instance failure when logging in, Instance failure when entering map portal in hideout, I don't know what else to do.
