But after i dead, i cameback to map. My unique flask do not recharge.
My map no flask modifiers, no expeditions don't have any extra contents.

Searing exarch altar effect:

(20–40)% increased Flask Charges used
(40–60)% reduced Flask Effect Duration
Some elaboration:

A flask appears empty if you do not have enough charges to activate it, even if the flask is at full charges. Your flask there is at full charges, but cannot be used because it requires more charges to activate than it holds at full.

Effects not on the flask that affect charge usage don't appear on the flask's tooltip. To observe that "increased flask charges used" is the issue, activate one of your usable flasks, and compare the charges the flask says it uses to the actual number of charges spent.

Once you activate an Eater/Exarch altar, the effects you chose are in effect as long as you're in that map.

Your flasks are full, they won't gain more than their max charges.

The problem appeared before you died and caused your death, but you didn't notice until you reentered.

Above, I explained how the "% increased flask charges used" modifier affected you and how to observe it.
