Enemies not taking reflect damage

It seems that in my testing enemies will not take reflected damage quite frequently.

I have looked over the mechanics page for reflect and it can be dodged, evaded, and blocked.

I have looked at the forum at GGG Mark's posts about reflect and found no resolution.

I have tested this in The City of Sarn and the Coast.

Testing materials:
-Marauder (no Ascendancy)
-Tabula Rasa
-Advancing Fortress
-springleaf
-Guard captain (City of Sarn)
-Mobs in the coast

Tree:
-unwavering stance (so I cannot evade)
-glancing blows (so I take damage even on block)
-resolute technique (to ensure that reflected damage cannot be evaded)
-spiked bulwark (for damage)

Method:
Advancing fortress and spiked bulwark should deal around 140 reflected damage to an enemy when I am hit by them. Naturally I have no chance to evade from unwavering stance and my hits cannot be evaded because of resolute technique. Just in case when I block I still take damage because of glancing blows.

Went down to the City of Sarn and Isolated the Guard Captain and let him hit me only about 50% of hits reflected damage back to him.

Okay he is a unique enemy and may have some mitigation modifiers or a chance to block or dodge.

Go to the coast and isolate melee mobs and the same result only about 50%-75% of hits from those mobs resulted in them taking any reflected damage.

Is this a bug, did I miss Something or do mobs have a chance to mitigate reflected damage somehow, because it seems highly unlikely that coast mobs should have the ability to mitigate 140 damage.
