Searing Exarch Altars not spawning correctly

Playing through Searing Exarch influenced maps, when you kill enough enemies to spawn an altar, the game sometimes does the "whoosh" effect that should spawn the altar, but the altar doesn't appear.

This has put my Searing Exarch Altar progress in a state where the Searing Exarch is not interested in any map less than T16, and I only have 5 of 10 altar activations. I assure you I didn't forget to click, or skip, any spawned altars. The altars didn't spawn to allow me to click, not to mention it should not really be possible to set the map device to Searing Exarch influence, and not get 10 alters as you progress from when you can first set Searing Exarch influence to Searing Exarch not being interested in anything less than a T16 map.

Anyhow, after playing through a T16 Searing influenced map last night, I managed to get to 8 of 10 Searing Exarch altars. But, playing through the map was highly annoying to the point of being upsetting. The first two "whoosh" triggers in the T16, the altars did not spawn (..., really?). But as I neared full map clear, a third "whoosh" triggered, and low and behold, 3 Searing Exarch altars spawned, all at once, all right on top of each other.

One, this should not happen. Two, had I not done a full clear and quit before hunting down every last enemy (which typically I wouldn't do as it's an inefficient use of time), this would have been yet another Searing Exarch influenced map where I got ZERO altars, when there should have been two long before nearing full clear.

Please fix this.

I'm getting quite tired of choosing Searing Exarch influence and having ZERO altars spawn in the map.

Zero altars. Zero exarch currency. Zero exarch influenced items.

Searing Exarch is a complete waste of time compared to Eater of Worlds.
