Not getting completion on Cortex/Chayula (The Feared)

Trying to complete The Feared and have done everything but cortex and chayula.

Just ran cortex without getting complete.
Also tried Chayula, Didn't get complete on that one eighter.

I've allready got complete on the other 3 needed.

*It was in my hideout
*It was a normal map, not from kirac if that matters


Is this a known bug or whats up?
Anyway i can solve this?

did you die and/or go to hideout during death animation?
did you select the maven witness button or leave it on one of the eldritch influences?

some completions don't update until you leave the instance as well.
