Suddenly huge stuttering on High end PC

I've been trying to deal with this since the start of 3.17, that's when it started for me.

The only thing that's made any difference for me is playing in full screen, as I've always played in windowed borderless but effects like flame bearers or volatile flameblood render way too late to dodge them

edit: nevermind, the stuttering is horrible on fullscreen too

I've been dealing with this as well, I have done all the things to try and resolve it. I have a blazing fast PC, and don't have this issue on any other game including those far beyond POE's requirements.

I have noticed that if there is anything that is also using the video card, this is when the issue occurs. (yes I shut off hardware acc. on the browser)

In the past, I have used older video card drivers, but this is a long process.

If I close down everything but path, sometimes it runs good. I have found that Path of building, twtich, and chrome trigger issues most often.

TL;DR: Something with the video card/driver IMO

I changed the Renderer in game from Direct X 11 to Vulkan and it fixed all stuttering problems for me, hope it helps for others. <3

FYI... I was dealing with the same issues switched to Vulkan and it actually made things worse. I changed to Dx12 Beta and everything started running smooth as hell and I run in 3440x1440,

Try this if u got nvidia control panel. might not work but worth a try.
Same here (didn't have this problem with Vulcan until 3.19. - didn't play 3.18).

This stuttering is actually visible when one opens the performance metrics: periodic spikes (without or with an empty/full screen) - it has NOTHING to do with performance issues (as the OP already said) or (internet/server) lag.

Workaround for me: switch to (Windowed) Fullscreen and then revert back (or the other way around).

It seems to me that it has something to do with certain (AN?) on screen effects. But so far that's just a feeling. Their appearance might start the problem but their disappearance for certain doesn't stop it.

Today it happened a few times over (Labyrinth, then Belfry map). Yesterday it hit me rarely.

I'm also having this issue, and as BluMetal mentioned, switching between Bordreless Windowed -> Fullscreen -> Borderless Window does seem to (temporarily) fix it. What I notice happens is that CPU usage will suddenly jump, and stay very high (30ms or so) when it'll normally idle 2-5ms. Once this happens zoning out can sometimes fix it as well. It feels like an effect causes a background task or something that gets stuck.
Another thing to keep in mind here. If you are running Win 11, there has been an ongoing issue with something within that OS causing stuttering etc while gaming. It was supposedly fixed in a patch internally, but I do not know if it has gone live yet.
