I killed every single monsters in the map but Bameth did not appear.

Not a bug.
He is not a guaranteed spawn. The prophecy will just remain in your list until you actually do defeat him. "Programming today is a race between software engineers striving to build bigger and better idiot-proof programs, and the Universe trying to produce bigger and better idiots.
