Still happening for me too :( just had 3 in a row when zoning into a map/dying and going back to hideout.

"Failed connect to instance" Feels like its better now, maybe from 1-5 maps, to 1-10 maps before it happen. Its unstable after chapter 5.

Latency spikes still continues. Is some problem with servers?
I check my connection and its good and stable 10-20 ms but in game latency going time to time very high and game freeze for sec or two. | |
