the 'embrace madness' skill is supposed to apply poison , well no poison is being added

I think you still need to have a chance to inflict poison. Have you tried activating Herald of Agony while affected by Glorious Madness? Your hits should be able to inflict poison then, even if you have no physical or chaos base damage.
